東京海洋大学 証明書発行申込システムについてTokyo University of Marine Science and Technology: Application System for Issuing Certificates
- 本システムで申請できるのは卒業生の方のみです。在校生の方は大学ホームページの在校生の方向けページで確認してください。
誤って本システムで申請し、クレジットカード決済を完了した場合は、手数料等その他の代金について一切返金できません。

If you make an application using this system by mistake and complete the credit card payment, we will not be able to refund any fees or other charges. - 本システム利用にあたり、申込や証明書発行手数料払込の完了メールを通知するためにメールアドレスが必要となります。必ず事前にメールアドレスをご用意ください。
To use this site, a mail address is required to notify completion mail of application and certificate issuance fee payment. Be sure to have your e-mail address in advance. - 一旦お支払い手続きを完了された証明書発行手数料、郵便料金は、返還しません。Certificate issuance fee, postage, etc. once paid will not be refunded.
- 郵便事故などによる紛失・破損等については、責任を負いかねます。 We can not assume the responsibility for lost / damaged etc due to mail accident etc.
- 領収証の発行はできません。We can not issue a receipt.
証明書の発行のお申込みは、決済代行を委託している株式会社マックスコネクトでのお手続きとなります。【株式会社マックスコネクト】Certificate issuance fee will be settled by MAXCONNECT Co., Ltd.
証明書郵送先についてMailing of Certificates
日本郵便より、新型コロナウイルス感染症の世界的まん延に伴い、一部の国・地域にわたる国際郵便物の引き受けについての停止または配達遅延が発表されています。（詳しくは日本郵便のＷＥＢサイトでご確認ください）。それに伴いまして、現在一部地域の証明書発送は見合わせております。該当する国・地域への国際郵便が再開されるまでお待ちくださいますようお願いします。Due to the ongoing global coronavirus pandemic, Japan Post has announced delays or suspensions of delivery of international mail to some countries and regions. (For details, please check the Japan Post website.) For this reason, we are currently suspending the mailing of certificates to some regions. We ask for your patience until international delivery resumes to the affected countries and regions.
申込枚数の上限についてLimit to Number of Certificates per Application
証明書発行申込システムでは、1回のお申込みにおける申込枚数の上限は合計20通となります。21通以上希望する場合は、改めてお申込みください。The certificate application system has a limit of 20 certificates per single application. For 21 or more certificates, additional applications will be required.

証明書の発行 1通 300円￥300 per certificate
郵便料金について（返信用封筒は大学が用意します）Postal Charges (the University will provide reply envelopes)

一律 370円 ※申込枚数の上限は20通まで￥370 per reply (Limit of 20 certificates per application)

お支払い方法についてPayment Method
お支払いはクレジットカードのみとなります。Payment by credit card only. ご利用いただけるクレジットカード（※本人名義のものに限ります。）The name on the credit card must match that of the applicant.

- 本人確認書類についてApplicant Identification
お申し込み時に本人確認書類（運転免許証・健康保険証（被保険者等記号・番号等にマスキングを施すこと）・パスポート等）の画像データをインターネット経由でアップロードいただきます。
携帯電話やデジタルカメラ、スキャナー等にて本人確認書類の画像データを予めご準備ください。At the time of application, please upload image data of documents being used to confirm your identity (e.g. driver's license, health insurance card (masking patient codes/numbers, etc.), passport, or similar) via the Internet. ※ 下記ファイル形式で画像データを保存ください。 (Notes) Save the data in one of the following formats: 画像データ形式： JPG・PDF Image data format: JPG, PDF ファイル容量： 1ファイル 300KB以下 File size: 300 KB or less per file ※ 運転免許証など、裏面に現在のご住所が記載されている場合は、裏面の画像データも作成ください。 (Notes) If your current address is shown on the reverse side of the identification documents (such as a driver's license), include an image of the reverse side. ※ 画像データが不鮮明な場合はお申し込み不備となりますので、ご注意ください。 (Notes)Your application cannot be processed if the image data is unclear.- 推奨環境はパソコンとなりますが、ご利用の機種、端末、OS、ブラウザなどの環境によっては動作に制約がある場合がございます。あらかじめご了承くださいWe recommend using a personal computer for the application.
